The Air Force currently uses two EPR forms. The AF Form 910 (Figures 1 and 2) is used to evaluate the performance of all enlisted members in the ranks of AB-TSgt. The AF Form 911 (Figures 3 and 4) is used to evaluate senior NCOs, MSgt-CMSgt.
